Governance is a critical building block of the health system, with the overarching function of ensuring that strategic policy frameworks exist and are combined with intelligence and oversight, collaboration and coalition-building, regulation, system design and accountability. This report aims to describe the challenges faced in the health financing system related to reviewing and decision-making for the Program of Medical Guarantees (PMG), and seeks to provide tangible short- and medium-term actions to take towards strengthening the governance of the PMG. These include (1) establishing a PMG Governance Board; (2) unbundling the PMG into clear processes linked to the budget, financing mechanisms, benefits and specifications of the PMG; and (3) defining the key decision responsibilities and deadlines in the annual PMG cycle, codified in regulations.
